§ 129B.03  APPLICATION FOR LICENSE.
   (A)   Applications for pedicab business licenses shall be made to the City Clerk on a form provided by the City Clerk. In addition to other information required by this chapter, the application shall contain the following information:
      (1)   The name and address of the applicant.
      (2)   The applicant’s work or corporate history, limited liability company history, or partnership history for the past five years. If the applicant is a corporation, limited liability company or partnership, the information shall also be given for the chief administrative officer who will be in charge of the LaSalle operations. The corporation shall list all its officers and principal business operated by the entity. If the corporation or limited liability company is a foreign entity, a copy of the certificate to do business in the state shall be provided.
      (3)   A list of all felony and misdemeanor convictions and/or orders of court supervision on misdemeanors by the applicant for the past ten years.
      (4)   A signed consent in regard to the City of LaSalle obtaining a police background check and a credit check if deemed appropriate in the discretion of the Fire Chief, Chief of Police, and/or Director of Economic Development.
      (5)   Each pedicab shall be operated by the holder of a valid Illinois driver’s license and must have that license at all times while operating the pedicab.
      (6)   A schedule of rates and charges to be made to passengers, which rates shall not be changed without 30 days’ prior notice to the City Clerk of the City of LaSalle, with all rates to be subject to the approval of the City Council of the City of LaSalle.
   (B)   The applications shall be verified under oath and include a written agreement by the applicant to operate the business, if licensed, strictly in accordance with this chapter, and indemnify and hold harmless the city for all judgments, liens and expenses arising out of the operations permitted by the license. Additionally, in the event that the applicant is a corporation, limited liability company and/or a partnership, the application shall be cosigned jointly and severally by an individual principal of the entity.
