Application for a distributor’s license shall be made to the City Clerk of the City of LaSalle and shall set forth the full name and address of the applicant or applicants, or the names and addresses of the partners if the applicant is a partnership or the names and addresses of the officers of the corporation if the applicant is a corporation, the present or last occupation of the applicant at the time of the filing of the application, a statement as to whether or not the applicant has been convicted of any felony or of pandering, gambling, operating a gambling device, operating a gaming house or being a keeper of a house of ill-fame or any other misdemeanor, and a statement as to whether or not the applicant is at the time of making the application keeping or is in any way connected with a gaming house or house of ill-fame.
