§ 110.31  REPORT.
   Whenever an inspection of a retail food establishment is conducted, the findings shall be recorded on an inspection report form approved by the Health Officer and the state Department of Health.  The inspection report form shall summarize findings on the basis of critical and non-critical violations, as approved by the state Department of Health.  Each occurrence of a violation will be recorded as a separate infraction, and the total number of critical and non-critical violations will be indicated on the inspection report.
