§ 110.06 APPLICATION.
   (A)   Every application for a license shall be made to the City Clerk-Treasurer on forms supplied by the city. Application forms may be obtained from the City Clerk-Treasurer upon payment of the required application fee. The application shall:
      (1)   State the applicant’s true and correct legal name, address, and telephone number;
      (2)   Identify the type license requested;
      (3)   Identify the premises at which the licensed activity will occur, if any;
      (4)   Identify any other business operated from the same premises;
      (5)   Identify the length of time for which the license is desired;
      (6)   Identify all persons associated with the licensed activity by name and address;
      (7)   Identify the type of thing or things to be sold;
      (8)   Identify the place or places of residence or principal place of business of the applicant for the past five years; and
      (9)   Identify the applicant’s state sales tax number.
   (B)   Applications shall be accompanied by the required surety bond. Every application shall bear the written approval of the Chief of Police after investigation. All applications shall be submitted to the City Council and a license may be issued by the City Clerk-Treasurer only after approval by the City Council.
