§ 111.02 APPLICATION.
   No receipt shall be issued except on written application of the person applying for it. It shall be the duty of the City Clerk, before issuing a receipt based wholly or in part on property valuation, capacity, number of persons employed, or any other contingency, to require the person applying for the receipt to file with him/her an affidavit giving full and complete information thereof. The applications and affidavit required by this section shall be retained as a part of the records of the office of the City Clerk.
