Within 30 days from the effective date of this chapter, the grantee shall file with the County Council a written statement in the following form signed in its name and behalf:
"To the County Council of the County of Lake, Indiana:
For itself, its successors and assigns, US CABLE hereby accepts the attached ordinance and agrees to be bound by all of its terms, conditions, and provisions."
