Should a certificate of number or registration expiration decal become lost, destroyed, or mutilated beyond legibility, the owner of the motorboat or sailboat shall make application to the Department for the replacement of the certificate or decal, giving his or her name, address, and the number of his or her boat and shall pay to the Department a fee of $5.
