§ 72.43 APPLICATION FORM FOR PARTICIPATION.
   (A)   The application form for participation shall be the form developed by the County Engineer for the efficient administration and review of the applications of associations applying for consideration for participation in the County Snowmobile Signing Accommodation Program.
   (B)   The form shall contain, at a minimum, the following information:
      (1)   Information to be supplied by the association:
         (a)   Name of the association;
         (b)   Address of the association;
         (c)   Name of the association president;
         (d)   Name, address, and phone number of the association coordinator;
         (e)   Signature and date of signature of the association coordinator;
         (f)   Indication if the application is for renewal of a county Snowmobile Signing Accommodation Program Agreement;
         (g)   A description of its desired choices of connecting sections and/or cross-connections. Each connecting section and cross-connection shall be on a separate page to enable the application to be considered separately in parts. Each description shall indicate the proposed direction of travel in relation to the vehicular traffic flow upon the adjacent roadway. Each description shall explicitly locate all crossings of the county highway;
         (h)   A listing of all locations that require operation of snowmobiles within ten feet of the roadway to avoid obstructions or hazardous terrain;
         (i)   The snowmobile trails that the association is involved in at each end of the connecting section or cross-connection which includes their approximate length and destination; and
         (j)   The written endorsement of all municipal corporate bodies, or their designees, for municipalities through which the desired connecting sections and/or cross-connections traverse.
      (2)   Information to be assembled by the Lake County Division of Transportation for the submitted applications:
         (a)   Date received;
         (b)   An indication if the proposed connecting sections and cross-connections are county highways or not;
         (c)   An indication if the proper municipal endorsements have been obtained;
         (d)   An indication if the application has been judged by the Lake County Division of Transportation to be complete;
         (e)   An indication if the particular proposed connecting sections and cross-connections are unavailable due to being proposed for use by another association;
         (f)   An indication if the proposed connecting sections and cross-connections are unavailable due to conflicting ongoing or pending highway maintenance or construction, or to conflicting ongoing or pending utility maintenance or construction; and
         (g)   An indication of where snowmobile signing, if any, is to be considered to be placed by the Division of Transportation on those connecting sections and cross-connections.
