For the purpose of this subchapter, the following definitions shall apply.
BOARD OF HEALTH. The Board of Health of the County of Lake.
HEALTH DEPARTMENT. The Lake County Health Department and Community Health Center.
HOME HEALTH AGENCY or AGENCY. The Health Department, which is responsible for providing the “home health service” in accordance with the certification standards established by the Illinois Department of Public Health.
HOME HEALTH SERVICES.
(1) Those items and services furnished to individuals, under the care of a physician, on a visiting basis in a place of residences used as the individual’s home.
(2) These services are provided by the home health agency under a plan, established and periodically reviewed by the patient’s physician, for furnishing such items and services to an individual on a part-time or intermittent basis and includes nursing care provided by or under the supervision of a registered professional nurse; physical, occupational, or speech therapy, medical social work, and home health aide.
