A. Application: Any person desiring a license for any such wagon or vehicle shall file an application with the village clerk upon a form to be provided by him, setting forth the following:
1. The name and address of the applicant;
2. A description of the wagon or vehicle for which the license is desired;
3. The place where such wagon, or vehicle for which the license is desired, is usually kept when not in use and the place where such animal or animals drawing such wagon or vehicle is or are to be kept;
4. The number and kinds of other wagons or vehicles kept by said applicant at such place;
5. In case the application is made for a license for a motor vehicle, whether or not such motor vehicle is used or is to be used for pleasure; and
6. Such other information as may be prescribed.
B. Issuance Of License: Upon the payment by the applicant of the license fee hereinafter provided to the village clerk, the village clerk shall issue or cause to be issued a license authorizing the use of such wagons or vehicles within the village until the expiration of such license.
