(a) All full-time employees, their spouses and dependent children under the age of twenty-six (26) shall be entitled to participate in the medical plan offered by the Village. The Village shall pay an amount not to exceed eighty-five percent (85%) of the premium up to a maximum of Two Thousand Five Hundred Dollars ($2,500.00) per employee per month for each full-time employee who elects to participate in the Medical Plan. The remaining fifteen (15) percent shall be the responsibility of the employee and shall be taken out of the employee's pay by payroll deduction.
(b) Employees shall be notified thirty (30) days in advance of any rate increase greater than the Village's cost limit.
(c) Vision and Dental Insurance is provided by the Village paying an amount that shall not exceed eighty-five percent (85%) of the premium. The remaining fifteen percent (15%) shall be the responsibility of the employee and shall be taken out of the employee's pay by payroll deduction
(d) Any full-time employee eligible to enroll in hospitalization and major medical plan insurance provided by the Village can waive said coverage and execute the appropriate document holding the Village absolutely harmless from any liability.
(e) The Village shall offer a reimbursement to employees who are required to enroll in Medicare and who elect not to participate in the Medical Plan in an amount not to exceed the amount that would be otherwise be paid pursuant to this Paragraph upon proof of payment of the Medicare Premium.
(f) All full-time employees shall receive life insurance coverage in the amount of twenty-five thousand dollars ($25,000.00).
