§ 114.39 CONDITIONS GOVERNING ISSUANCE OF.
   (A)   The license shall be issued only to persons who have completed 400 hours of certified therapeutic massage training from a bona fide school that has been properly accredited, or has one year of experience practicing massage therapy as established by an affidavit and can document within two years of obtaining the license that he or she has completed 400 hours of certified therapeutic massage training from a bona fide school. If no such documentation can be established at the time of license renewal, the license shall not be renewed and the person who received the license based upon experience is precluded from receiving a license in the future unless the person has the requisite certified hours.
   (B)   The license shall be issued only to persons of good moral character and repute.
   (C)   The license shall be issued only to persons free of convictions for offenses which involve moral wrongdoing or which relate directly to such person's ability, capacity, or fitness to perform the duties and discharge the responsibilities of the occupation.
   (D)   The license shall not be issued to persons who have, within one year prior to the date of application, been denied licensure; or who have had his or her license revoked or surrendered in or by any political subdivision, municipality or by the State of Minnesota.
   (E)   The license shall be issued only to applicants who have fully and truthfully answered all of the information requested in the application and have paid the full license and investigation fee.
   (F)   The license shall be issued only to persons 18 years of age or older.
