An application for a massage therapist license shall be made only on forms provided by the city. The application shall contain the following information together with any other information the city may require:
(A) Evidence of the applicant's educational qualifications, including proof of receiving at least 400 hours of certified therapeutic massage training from a bona fide school that has been properly accredited, or documentation of one year of experience practicing massage therapy as established by an affidavit and can document within two years of obtaining the license that he or she has completed 400 hours of certified therapeutic massage training from a bona fide school.
(B) Whether the applicant has ever been convicted of a crime or offense other than a traffic offense and, if so, information as to the time, place and nature of such crime or offense.
