Verified applications for a business license shall be made by the owner of the business, trade or profession, or by a designated agent, upon a form to be furnished by the city clerk. Such application shall contain the following information:
A. The full name of the owner of the business, trade or profession;
B. The mailing address of the owner of the business, trade or profession;
C. The physical address of any and all locations within the city at which such business, trade or profession is to be conducted;
D. The name and address of an agent of the business, trade or profession, who is designated by the owner as authorized to receive any notices permitted or required by law to be provided to the business, trade or profession;
E. A description of the nature and purpose of the business, trade or profession to be licensed; and
F. The signature of the owner of the business, trade or profession, or the signature of the owner's designated agent. (Ord. 126, 12-6-1999)
