§ 70.26 DEFINITIONS.
   For the purpose of this subchapter the following definitions shall apply unless the context clearly indicates or requires a different meaning.
   “ACCIDENT REPORT.” Shall consist of the following information.
      (1)   The name and address of the owner and operator of any vehicle involved in an accident.
      (2)   The license number and description of any such vehicle involved in said accident.
      (3)   The time and place such accident occurred.
      (4)   The names and addresses of any persons injured or fatally injured in such accident.
      (5)   The names and addresses of any persons who were witnesses to such accident.
   “QUALIFIED INDIVIDUAL.” Any person who has sustained any loss or damage, by reason of the injury or death of any person or damage to property caused by or resulting from the operation, maintenance or use of any vehicle on the public streets or highways located in or about the city, or that person’s duly authorized agent or attorney.
