§ 111.21 APPLICATION; CONTENTS.
   Applications for approval by the Township Board of an on-premises license shall be made to the Township Board in writing, signed by the applicant (if an individual) or by a duly authorized agent (if a partnership, limited liability company, corporation or other entity), verified by oath or affidavit, and shall be accompanied by all necessary documents evidencing compliance with or providing assurances for future compliance with all requirements of this ordinance as well as the Michigan Liquor Control Code, as amended, M.C.L.A. §§ 436.1101 et seq. All applications shall contain, at a minimum, the following information:
   (A)   The name, age and address of the applicant in the case of an individual; or, in the case of an entity, all of the owners, other persons entitled to profits or distributions from the entity, all persons having a right to participate in the management of the enterprise, and certified copies of the entity’s organizational documents including by way of illustration, the following: articles, bylaws, partnership agreement, and operating agreement;
   (B)   The contact person for the applicant, including address and telephone number;
   (C)   A description of the premises location and business to be operating under the license;
   (D)   A statement that the applicant will not violate any local, state or federal laws and regulations in the conduct of the business;
   (E)   Plans showing all structures and premises, including the specific areas where the license is to be utilized;
   (F)   A complete copy of the Application for New License, Permits or Transfer of Ownership or Interest, being Form LCC-3011, or any replacement form of the Liquor Control Commission and any attachments submitted to the Michigan Liquor Control Commission.
   (G)   A completed copy of any investigations, inspections, reports, correspondence, and communications issued to the licensee applicant by the Michigan Liquor Control Commission;
   (H)   A completed copy of the Police Investigation Recommendation, being Form LC 1800, or any replacement form of the Michigan Liquor Control Commission.
   (I)   Any other information deemed necessary by the township to adequately review the application as provided by this section and authorized by law.
