§ 834.03  LICENSE APPLICATION.
   An application for the license shall be made to the Director of Public Safety on forms to be prescribed by him or her and shall state the name and address of the applicant, if an individual, and, if a firm or partnership, the names, residences and occupations of the manager and each member of the firm or partnership, and, if a corporation, the names of its officers and managers, and such other information as the Director may reasonably require.
   The applicant shall attach to the application a floor plan, drawn to scale, showing the proposed placement of each amusement device on the applicant's premises. All regular and emergency exits shall be clearly identified on the floor plan.
   The application shall contain the name or names and addresses of all persons who will be acting as supervisors of the game room, and the Director shall be notified in writing, within 24 hours, of any change in supervisory personnel.
   The application shall be rejected if it is found that any person named in the application has been convicted of a felony or a misdemeanor involving moral turpitude or that the game room sought to be licensed does not comply with any zoning, building, health or fire regulation applicable thereto.
