§ 1066.01  FEES FOR NONRESIDENTS.
   (a)   The city will provide ambulance and emergency rescue squad services to residents and the city shall only bill residents of Kirtland and the Village of Kirtland Hills and their insurers, including the Federal Health Care Programs of Medicare and Medicaid, for the services provided but only to the extent of their insurance coverage, i.e. there will be no out of pocket costs to the residents of the City of Kirtland and the Village of Kirtland Hills. The city shall treat the revenues received from Kirtland local taxes and the Kirtland Hills Fire/EMS contract as payment of any otherwise applicable co-payments and deductibles that would otherwise be due from the residents of the City of Kirtland and the Village of Kirtland Hills. The city will not charge or collect any amount of money in excess of the usual, customary and reasonable (UCR) payments, including any amounts received from Medicare and/or any Medicare Supplemental Insurance carriers and/or Medicaid.
   (b)   The city will provide transport and emergency medical service to the nonresident user and the city will request full payment from the non-resident user at the rate established by the Mayor for the level of service provided. The city shall bill the insurance company, if any, and/or the non-resident for the service as required by this section.
   (c)   The levels of service are as follows:
      (1)   ALS 1;
      (2)   ALS 2;
      (3)   BLS;
      (4)   Mileage ALS; and
      (5)   Mileage BLS.
   (d)   The Mayor is authorized to adjust the charges annually after considering the actual cost of providing these services and after consideration of inflationary adjustments allowed by the Federal Government.
   (e)   The Mayor is authorized to enter into an agreement with a company that will provide electronic claims filing systems for the purpose of collecting insurance proceeds indicated in subsection (c) hereof or as modified thereafter.
   (f)   That the Mayor and the Finance Director are authorized to establish guidelines and procedures to assist the indigent and the financially needy that are unable to pay for the rescue squad services.
   (g)   That all funds collected pursuant to this section shall be deposited to the Major Capital Equipment Fund.
