§ 110.04 PERMIT APPLICATION.
   (A)   The Chief of Police shall have general authority to establish procedures, forms, and reasonable requirements for solicitor permit applications and shall be authorized to adapt the application forms and requirements, as may be required from time to time, due to changes in law and/or background screening capabilities, so long as the permit requirements remain objective and uniformly applied to applicants.
   (B)   Application for a solicitor's permit shall be made in writing, notarized, on a form provided by and filed with the village police department, to include the following information:
      (1)   An attached printout indicating satisfactory completion of the Illinois State Police fingerprint background check process;
      (2)   The applicant's full name, all addresses of applicant's residence during the past two years, the length of time of the applicant's residence at each address, and the applicant's business address if other than their current residence address;
      (3)   The applicant's place and date of birth, social security number, driver's license number and state of issuance or current state or federal identification card and physical description;
      (4)   The name and address of the business or other person or entity by whom the applicant is employed or whom the applicant represents, and the initial date of such employment or representation;
      (5)   Description sufficient for identification of the subject matter of the soliciting which the applicant will engage in;
      (6)   The dates for which the applicant seeks a solicitor's permit;
      (7)   Information, including the date(s), regarding any prior application or use of a solicitor's permit by the applicant in the village, and any revocation of a previously issued solicitor's permit by the village;
      (8)   Information regarding any conviction of any felony or misdemeanor involving dishonesty, theft, fraud, false statements, a threat to public safety, or a crime involving sex offenses or offenses involving bodily harm against persons under the laws of this state or any other state or federal law of the United States within five years of the date of the permit application; and
      (9)   Information as to whether a solicitation permit has been previously issued to the applicant by any other jurisdiction, and if so, whether the permit was ever revoked.
   (C)   Each application shall be accompanied by a nonrefundable fee of $50 for the investigation and administration of the provisions of this subchapter and the payment of the amount required to complete fingerprinting through the state police or other entity. If the applicant has previously been cited for soliciting without a permit in the village, this non-refundable application fee is $100.
   (D)   The Chief of Police shall cause to be kept in his or her offices an accurate record of every application received and acted upon, together with all other information and data pertaining thereto, and copies of all solicitation permits issued under the provisions of this subchapter. Applications for permits shall be numbered in consecutive order, as filed, and every permit issued and any renewal thereof shall be identified with the duplicate number of the application upon which it was issued.
