(A) Any person desiring to make any connection with the system and plant, or use water therefrom, shall first make application to the Superintendent for that purpose upon a blank form or forms to be furnished by him or her. Every applicant shall state specifically and in detail the place, location and kind of connection and appendages sought, the purpose for which the water is to be used, and all the plumbing and plumbing work in any manner or way connected therewith.
(B) It shall be the duty of the plumber installing the work to report to the Superintendent everything done in connection therewith and any departure from or violation of the permit. Every person so making the connection with the system or plant or using water therefrom shall make connection and use the water subject at all times to the provisions and conditions of the ordinances of the village in force or to become in force at any time during the period the connections are maintained or the water is used, and shall in all respects conform to the provisions and conditions of this chapter.
