A certificate of announcement shall be filed, on behalf of all candidates for city offices, with the City Recorder. Such certificate shall show the office for which the candidate is filing, the name of the ticket he or she represents, if any, his or her residence, number of ward and a statement of eligibility to hold said office. Certificates of announcement forms shall be furnished to all candidates by the City Recorder.
