Unless otherwise provided, an application for a business license shall state the following information:
(A) Name, address, date of birth and telephone number of applicant or applicants;
(B) Trade name, if any, under which license is to be exercised;
(C) If partnership, the name, address, date of birth and telephone number of each partner;
(D) If a corporation, the name, address, date of birth and telephone numbers of the officers;
(E) Place of business or location where the license is to be exercised and tax license number for the business;
(F) Description of the activity or nature of business to be carried on under the license;
(G) The information as required by the Board of Trustees showing the applicant is entitled to the license and that he or she is the proper person, and the place a proper place for the exercise thereof;
(H) The information as required by ordinance as may be necessary for determination of the amount of the license fee; and
(I) Furnish information in the case of security police, the owner of the business must provide the local law enforcement with a list of all personnel who will be carrying firearms within the town. This list must be up-dated throughout the year as personnel are added or deleted from its employ.
