A. Application for renewal of licenses and for original issuance of licenses shall be filed with the City Clerk on or before the first Monday in the month of May of each year and shall be accompanied by the proper fee for such license as determined by this chapter and, furthermore, shall be accompanied by a statement by the applicant, under oath, stating that he is qualified and eligible under the requirements specified in 235 Illinois Compiled Statutes 5/6-2 to apply for and receive such a license in the event such license might be granted. B. The applicant shall also submit proof that applicant has presented himself at the County Sheriff's Office and submitted to fingerprinting for identification purposes, and that the Sheriff`s Department has done a background check for the proposed license which shows that issuance of the license to the applicant is not prohibited under the provisions of 235 Illinois Compiled Statutes 5/6-2.
