Any person, other than those seeking multiple water connections, who desires or is required to secure a new connection to the town water system shall file with the Water Department for each such connection a written and signed connection application in substantially the following form:
KANARRAVILLE, UTAH APPLICATION FOR WATER CONNECTION TO THE TOWN OF KANARRAVILLE, UTAH As Property Owner, I hereby apply to the Town of Kanarraville for permission to connect my premises at ___________________________________ to the town’s water system and hereby agree to the following: 1. (a) Kanarraville Town gives permission for the above stated water connection from its water main to the water meter, up to my property line, or to the meter if it’s installed within my property. I agree to pay Kanarraville Town an Impact Fee of $3,500, and a $2,500 Connection Fee. The Connection Fee includes the following costs associated with the water connection to the main line (meter, meter barrel, lid, saddle, parts, excavator fees, etc.) Any additional costs associated with reaching the connecting point on the main line for the requested meter may be charged in addition to the $2,500 Connection Fee (such as: boring under or digging through the road, road repair, additional piping, etc.) Separate checks in the amounts of $3,500 made out to “Kanarraville Town Impact Fee”, and $2,500 made out to “Kanarraville Town” must be paid to the Kanarraville Town Treasurer with the completed Water Connection Application. In addition, the work of extending the water connection from the meter to the place at which the water is to be used shall be the responsibility and cost of the Resident. (b) The connection so made by the town, including the meter, shall remain the property of the town, and the town shall have access thereto at all times. 2. The location of the meter, whether on my premises, or at some point near my premises, may be decided solely by the Town of Kanarraville, Utah. 3. Before making connection with the water system, I shall cause the plumbing upon my premises to be inspected by the town and if the plumbing is not approved, I will cause the plumbing to be rectified at my own expense to meeting the requirements of the town or of any other governmental agency having jurisdiction to regulate the water system within the town. 4. I will be bound by the rules, regulations, resolutions or ordinances enacted now or hereafter by the Town of Kanarraville, Utah, applicable to the town’s water system. 5. This water connection will be used for culinary purposes. The town shall have free access to the lines and meters installed under this agreement and, at reasonable times, through my property if necessary. 6. Water will be turned on as soon as the meter has been installed, fees have been paid, and all required paperwork has been filed with the Town Treasurer, unless the Town Treasurer has been notified otherwise. 7. Billing will commence as soon as the water has been turned on, and will continue until the Primary Account Holder has informed the Town Treasurer otherwise. 8. As Property Owner (landlord) I understand that I am the Primary Account Holder on the water account and will be solely responsible for all charges and fees unless I have notified the Town Treasurer to close my account and discontinue service.
