§ 123.03 APPLICATION FOR TAXICAB LICENSE.
   An application for a taxicab license shall be filed with the City Clerk upon forms provided, and the application shall furnish the following:
   (A)   The name, date of birth, and address of the applicant.
   (B)   The financial status of the applicant, including the amounts of all unpaid judgments against the applicant and the nature of the transaction or acts giving rise to the judgment.
   (C)   The experience of the applicant in the transportation of passengers.
   (D)   Any facts which the applicant believes tend to prove that public convenience and necessity require the granting of a license.
   (E)   The number of vehicles to be operated or controlled by the applicant, the location of proposed depots and terminals, the owner of the vehicles, and the license number of each vehicle.
   (F)   The color scheme or insignia to be used to designate the vehicle or vehicles of the applicant.
   (G)   A statement signed by the applicant indicating the applicant’s understanding that falsification of any item on the application is sufficient reason upon which to base a denial of the license or termination thereof.
   (H)   Such further information as the Council may require, including documentation concerning the officers, owners of interest, partners, and the like, of the applicant business entity.
