§ 96.127 APPLICATION FOR PERMIT.

Any person desiring a permit to install or replace any culvert shall file an application therefor with the City Clerk upon a form to be provided for that purpose. The application and the permit issued pursuant thereto shall be on the same form which shall be substantially as outlined in Appendix A of this chapter.
