(A) The license holder has an affirmative and continuing requirement to update the original and all renewal applications. As a result, the license holder must inform the Health Officer of any changes in the information listed in these applications within 30 days.
(B) Failure to comply with their requirements of this section, or knowingly furnishing false information on the original or renewal applications shall be grounds for immediate suspension or revocation of any license issued pursuant to this chapter.
