(A) Annual renewal of permits shall be required for continued operation of the establishment. Any person desiring to renew a permit shall make a written application on forms provided by the Health Department.
(B) All permit fees for the annual renewal of permits are due January 31 of the calendar year. Persons failing to submit the appropriate fee and renewal application by the above renewal due date shall be assessed a past due payment penalty fee of $50 per week and the appropriate permit fee. Failure to submit the total fee and application by the above-described renewal date may result in a lapse of the permit.
