(A) Annual renewal of licenses shall be required for continued operation of the establishment.
(B) Any person desiring to renew a license shall make written application on forms provided by the Health Department.
(C) All registration fees for the annual renewal of licenses are due 15 days prior to the license expiration date. Persons failing to submit the appropriate fee and renewal application by the above state renewal due date shall be assessed a late payment penalty fee of $50 in addition to the appropriate license fee. Failure to submit the total fee and application by the above described renewal date may result in a lapse of the license.
