The application for a license under this section shall be made on a form supplied by the city and shall include the following:
(A) The full name, current address, and home and business phone numbers of the applicant and of each employee of the business;
(B) A sketch or diagram showing the configuration of the premises, including a statement of total floor space occupied by the amusement arcade (the sketch or diagram need not be professionally prepared but must be drawn to a designated scale or drawn with marked dimensions of the interior of the premises to an accuracy of plus or minus 12 inches);
(C) The location of each machine or device to be licensed;
(D) The primary type of business to be operated on the licensed premises;
(E) The number of machines or devices to be licensed at the location; and
(F) Any additional information the city may reasonably request.
