§ 74.05 GOLF CART REGISTRATION REQUIRED.
   No person shall operate, or permit a golf cart owned or controlled by him or her to be operated, on any street or roadway in the City without having first registered the golf cart with, and obtained a license for the golf cart from, the City.
   (A)   Application for license. An application for a golf cart license shall be filed with the City Administrator upon forms provided by the City, and the application shall provide for furnishing of the following information: the full name and address of the applicant; the applicant’s date of birth; the state of issuance of the applicant’s current driver license; the number of the driver license; the name and address of the insurer providing vehicle liability insurance coverage; the name and address of the local insurance agent for the insurer; the number of applicant’s insurance policy covering the golf cart; the date of expiration of such insurance coverage; the year of manufacture, make, model, and identification number of the golf cart; the date of application; a statement that all information provided by the applicant on the application is true and that falsification of any item on the application is sufficient reason upon which to base a denial or revocation of the license; the applicant’s signature; and such further information as the City may reasonably require.
   (B)   Acceptance or denial. Within a reasonable time following the license application verification process, the City Administrator shall accept or deny the license application. If the City Administrator finds that the information on the application is accurate and that the applicant has paid all required fees, the City Administrator shall grant a license. Otherwise, the application shall be denied.
   (C)   Appeal of denial. If the application is denied, the applicant may request a personal appearance before the City Council to offer evidence why his or her application should be reconsidered and granted.
   (D)   Identification tag. The City shall issue a numbered identification tag which shall be displayed upon the rear exterior of the licensed golf cart at all times during the license period.
   (E)   Expiration. Licenses shall be issued for a period of five years.
