§ 56.03 ANNUAL REGISTRATION REQUIRED.
   (A)   Every entity that occupies right-of-way within the village shall register on January 1 of each year with the Community Development Director, providing the entity's name, address and regular business telephone and telecopy numbers, the name and e-mail address of one or more contact persons who can act on behalf of the entity in connection with emergencies involving the entity's facilities in the right-of-way and a 24-hour telephone number for each such person, and evidence of insurance as required in § 56.08 of this chapter, in the form of a certificate of insurance.
   (B)   Any entity not doing business within the village as of January 1 of any year shall be required to provide the information required by this section prior to commencing work in the right-of-way in that year, and on January 1 of each year thereafter regardless of its initial registration date.
   (C)   There shall be an annual, non-refundable registration fee of $100, to be paid at the time of the entity's registration. If an entity registers after the last day of June in a given calendar year, then the registration fee shall be one-half of the annual fee.
