The following information must be included with the application form before any license shall be granted. This information is to be used for a proper criminal history check and for administrative purposes.
(a) A list of names and dates of birth of all employees working in the City of Ionia.
(b) Two valid character references.
(c) A photocopy of legal picture identification, such as a driver's license.
(d) Type of advertising (if any).
(e) Authorization of the applicant to act as a representative from the person, firm or corporation for which the applicant is to act as such representative, as required by Michigan Compiled Laws 445.371-378.
(f) The applicant's State transient merchant license.
(g) A copy of the Michigan sales tax license.
(h) A statement from the owner of private property if the applicant wishes to sell on private property.
