Applicants for the license required by this chapter must file with the City Clerk a sworn application in writing, in duplicate, on a form to be furnished by the City Clerk, a copy of which follows the text of this chapter (Exhibit A), which form shall contain the following information:
(a) Full name of the applicant.
(b) Social Security number.
(c) Date and time of application.
(d) Date and place of birth.
(e) Driver's license number.
(f) Permanent address (legal).
(g) Local address (if any).
(h) Occupation.
(i) Home phone.
(j) Business address.
(k) Business phone.
(l) Title of person applying.
(m) Name of firm or corporation.
(n) Length of time for license, available only for two days, one week or six months.
(o) Previous places of business within the last six months.
(p) A list describing the article or articles to be sold or offered for sale.
(q) Value of goods for sale. This would be the total dollar amount of the merchandise for sale.
(r) Source of goods to be sold: from stock in possession, by sample, by direct sale or by taking orders for future delivery.
(s) Proposed method of vending: by foot, handcart, vehicle, etc. If merchandise is sold from a vehicle a description that includes the make, model, year and serial number.
(t) Present location of goods to be sold.
(u) Whether the applicant has ever been convicted of a crime or a violation of any City ordinance, and, if so, the nature of the offense and the punishment therefor.
