4-14-5: APPLICATION FOR PERMIT:
The application for a large assembly permit shall be made in writing and filed with either the Park and Recreation Department if a privately sponsored event is planned to be held in any City park, or the Police Department if the large assembly is to occur at any other location. Application for a large assembly permit shall be made no less than thirty (30) days in advance of the proposed date for the large assembly, upon forms supplied by the City. The application shall contain, the following information:
   A.    The contact information of the applicant, if an individual; all event partners if a partnership, and all officers of the corporation if the applicant is a corporation.
   B.    The address and legal description of all property which will be in any way used or occupied by the large assembly.
   C.    The names and addresses of all persons holding an equitable or legal interest in land, except mortgagees, where the proposed large assembly is to be conducted, together with a written statement of their approval of such use.
   D.    The names and addresses of all persons with a legal or equitable interest in the land which abuts the subject property.
   E.    A full description of the nature, purpose, and extent of the proposed large assembly. The event description shall include a description of all entertainment or activities attending the event, such as food service, alcohol service, the use and monitoring of inflatables, amusement rides, activities associated with a body of water, the areas where alcohol will be served, fireworks, stunts, or other exhibitions for the enjoyment of participants.
   F.    The maximum number of persons that the proposed large assembly may attract at any given time, together with the dates and hours during which the proposed large assembly will be conducted.
   G.    The full description and information on security plans to be utilized at the proposed large assembly by the applicant, including plans to mitigate access to alcohol by an underage person, if applicable.
   H.    A full description of plans for sound amplification and sound control to be utilized by the applicant.
   I.    Plans for any food or other concessions which will be provided or offered for sale, together with the name, address and licensing information of all concessionaires, catering services, food trucks, including licensed liquor establishments.
   J.    All other information which would be required by any City department required to review the permit application.
   K.    A traffic management and parking plan, including special requests for the bagging of no parking signs, if requested or required.
   L.    Permanent or temporary structures that will be utilized for the event, including but not limited to private homes, garages, sheds, or other outbuildings, tents of any size, portable toilets, or other structures.
   M.    All site clean-up from the event shall be complete within thirty six (36) hours from the termination of the large assembly event.
   N.    The nonrefundable permit review and investigation fee as provided in the City's published fee schedule. (Ord. 1365, 2-25-2019)
