§ 95.04 STANDARDS.
   (A)   Drawings and/or any additional information may be requested by the town.
   (B)   Street or alley: no cut to be less than 18 inches wide.
      (1)   Asphalt pavement:
         (a)   May be chisel cut with four-inch blade and then saw cut straight prior to replacement;
         (b)   Base and pavement materials shall be replaced identical to or better than original;
         (c)   All seams shall be tack coated and shall be ironed as necessary to provide a perfect joining of the patch with the surrounding pavement; and
         (d)   No cold mix asphalt is to be used except as a temporary patch.
      (2)   Concrete pavement:
         (a)   Shall be saw cut;
         (b)   Patch shall be at least 4000# concrete;
         (c)   Edges of adjoining pavement shall be coated with an epoxy adhesive intended for concrete patching; and
         (d)   Area shall be barricaded for proper curing time. Barricaded per division (J)(2)(c) through (J)(2)(e) below.
      (3)   Gravel pavement: all repairs shall be made with granular backfill (per division (I)(1) below).
   (C)   Curbs, rolled or straight, all to be saw cut.
      (1)   Asphalt: all materials to be equal to or better than original and formed to match original contour.
      (2)   Concrete:
         (a)   Area shall be excavated to depth to match mating curb; and
         (b)   All materials to be equal to or better than original and formed to match existing curb.
   (D)   Sidewalks, concrete:
      (1)   To be saw cut;
      (2)   Four inches thick in pedestrian area, minimum of six inches thick in vehicle area;
      (3)   All replacement concrete to be fiber reinforced; and
      (4)   Base shall be minimum two inches pea gravel or two inches sand compacted.
   (E)   Driveway: all to be saw cut:
      (1)   Asphalt:
         (a)   Minimum five inches base coat or match existing with one-inch top coat; and
         (b)   Backfill per division (I)(1) below.
      (2)   Concrete:
         (a)   Minimum six-inch fiber reinforced concrete; and
         (b)   Backfill per division (I)(1) below.
      (3)   Gravel:
         (a)   Shall be minimum of six inches of #73 stone and topped with two inches material equal to or better than original; and
         (b)   Backfill per division (I)(1) below.
   (F)   Lawn area:
      (1)   Shall be top soiled, fertilized, seeded, mulched or straw and restored to original contour. Watered as necessary for growth; and
      (2)   If settlement of more than four inches occurs within three years of date of closer of excavation, permit holder shall repair the area.
   (G)   Grade work: all finish grade contoured to adjoining area.
      (1)   Sodding:
         (a)   A minimum of two-inch good top soil to be applied prior to seeding; and
         (b)   Sod to be a good high quality blue grass.
      (2)   Seeding:
         (a)   A minimum of two-inch good top soil applied before seeding;
         (b)   Seed to be a high quality blue grass or high quality seed mix; and
         (c)   Mulch or straw cover and water as necessary to advance growth.
   (H)   Drainage:
      (1)   Swales/grades to follow land contour to provide original drainage; and
      (2)   Pipes/conduits to be placed per specs for proper size and location.
   (I)   Backfill, granular:
      (1)   All but top two feet shall be compacted lifts not to exceed 12 inches; and
      (2)   Top two feet shall be compacted #73 stone placed in maximum eight inches lifts.
   (J)   Cut and patch protection:
      (1)   Hot asphalt:
         (a)   Shall meet specs as prescribed;
         (b)   All joints, edges or exposed faces of curbing and pavement shall be cleaned and tack coated;
         (c)   Final surface shall be machine rolled with equipment standard to the industry; and
         (d)   Hand tamping not allowed.
      (2)   New concrete:
         (a)   Shall not be placed at temperature below 35°F;
         (b)   Shall be protected against excessive dehydration;
         (c)   Shall be closed to traffic a minimum of 48 hours;
         (d)   Three-quarter-inch steel plates shall be used to cover area; and
         (e)   Approved barricades and flashing lights shall be placed appropriately.
   (K)   Permit holder shall be responsible for all subcontractors work and materials used.
   (L)   Permit holder shall make provisions to maintain proper traffic flow at all times. For any road closings, a person must notify officials.
   (M)   Emergency notification.
      (1)   When threat to public safety exists, the town will immediately barricade the area. Any costs accrued must be paid by permit holder.
      (2)   Permit holder shall be notified by telephone or in person of the hazard.
   (N)   Permit holder shall be responsible for maintenance for a period of three years for area of public way where work was performed.
   (O)   Excavation on a state road easement within the town limits will require a state permit in addition to the local permit.
   (P)   The Town Manager may deny a permit application that for any reason is not in the best interest of the town. Applicant may appeal the decision in writing to the Town Council.
   (Q)   Trees in right of ways are required to be protected and/or replaced per tree. Tree damage may not be immediately noticeable so there will be a three-year maintenance period.
   (R)   Under no circumstances will a permit be issued before all required reviews have been conducted by appropriate town authorities.
