(A) Submission; information required. Ten copies of a preliminary plat shall be prepared and submitted to the Village Clerk. The plat shall include or have submitted with it the following information:
(1) A scale drawing of not more than 200 feet to the inch;
(2) The date, cardinal points, and a north arrow;
(3) The names, addresses, and telephone numbers of the proprietor and the engineer or surveyor who designed the subdivision;
(4) The proposed name of the subdivision which name shall not be the same or similar, as so to create confusion, with the name of other subdivisions within the county;
(5) The names, locations, and width, where applicable, of proposed streets, alleys, easements, parks, lot lines, existing, or proposed utilities, rights-of-way, and other proposed areas within the plat;
(6) Legal description of the land to be platted and its area in acres. The boundaries of the subdivision shall be described by metes and bounds and shall be drawn so as to be easily ascertainable in contrast to the remainder of the plat;
(7) A location map showing the relationship of the proposed plat to the surrounding area and connections with adjoining streets;
(8) Existing land use and zoning of the proposed subdivision and abutting tracts and any requests for changes in the same;
(9) A map of the entire area scheduled for development, including future streets if the proposed plat is portion of a large holding intended for subsequent development;
(10) Layout, number, and dimensions of lots. All lots shall be consecutively numbered;
(11) Location, dimension, and purpose of any land intended to be dedicated or set aside for public use or for the common use of property owners in the subdivision;
(12) The location of any flood plain areas;
(13) The location of any existing proposed sanitary sewers, water mains, storm drains, and other underground facilities within or adjacent to the proposed subdivision, including the location and dimensions of proposed street systems and easements therefor;
(14) Existing and proposed contours at two-foot elevations;
(15) Ground elevations at the intersections of all streets and at the extremities of the plat using NGVD, 1929 datum;
(16) Three copies of any proposed covenants and deed restrictions, or statement in writing that none is proposed. If common areas are to be reserved for use of the residents of the subdivision, copies of an agreement showing how the area will be maintained shall also be submitted; and
(17) A statement indicating the proposed use to which the subdivision will be put, along with a description of any type of residential buildings and number of dwelling units contemplated or the type of business or industry so as to reveal the effect of development of traffic, fire hazards, or congestion of population.

(B) Engineer, surveyor to prepare. The plat shall be prepared by a registered engineer or a registered land surveyor.
