Applications for permits shall be accompanied by the following:
(A) A plat plan to show the property owned or controlled by the applicant with reference to streets, highways and contiguous platted and unplatted areas.
(B) The city may require the dedication of any rights-of-way as may be deemed lacking, desirable and necessary, in the opinion of the City Traffic Engineer, and improvement of such rights-of-way as may be required by the City Traffic Engineer.
(C) Cross-sections to show elevation and grades at the final boundaries of excavation.
(D) A final grading plan to show the ground elevations of the land immediately adjacent to the site of the excavation and all abutting streets or roads.
(E) Upon completion of the excavation, its discontinuance or abandonment, and/or when there is a question that the excavation is in accordance with the plans approved, a topographical survey will be required showing elevations and cross-sections of the final boundaries of each excavation at not more than 100-foot intervals, and/or such other locations as may be required for determination of the adequacy of progress.
(F) The plans, maps, elevations and cross-sections required by this section shall be made by a surveyor-engineer registered as such by this state. Cost of said service shall be borne by the holder of the permit.
