A Qualified Provider must be prepared to comply with the following requirements:
(A) Complete and sign an application form provided by the City Manager or his/her designee;
(B) Provide the City Manager or his designee with any requested information, including, but not limited to location, maps, plans and specifications.
(C) Pay the nonrefundable License Agreement Application Fee as provided herein;
(D) Provide proof of financial responsibility by posting a bond as required hereunder;
(E) Provide a current certificate of insurance as provided hereunder;
(F) Provide evidence that all other necessary approvals from all appropriate City agencies have been obtained; and
(G) Otherwise comply with the terms and provisions of this chapter.
