(A) Every person desiring to engage in the occupation of an employment agency shall as a condition precedent to the issuance of a local business tax receipt, file with the Department of Financial Services, an application in writing under oath for local business tax receipt under this chapter, accompanied by a copy of a state license issued under F.S. Chapter 449.
(B) The application shall contain the following information:
(1) The name, address and place of residence of the applicant;
(2) The address of the place of business in which the agency shall be operated;
(3) The name or names and addresses of the persons who shall operate or be employed in the employment agency;
(4) The names and addresses of all persons who shall have any proprietary interest in the agency;
(5) The names and addresses of three persons as references residing in the city; and
(6) A statement of whether or not the applicant, or any person connected in any way with the agency, has operated or conducted an employment agency in any other city or town.

(C) The local business tax for an employment agency shall be $50 per annum.
