(A) Individual bond. Prior to the issuance of a permit, each individual shall file with his or her application a certificate of general liability insurance in the amount of $10,000, written by a duly qualified insurance company licensed to do business in the state.
(B) Blanket coverage. Prior to the issuance of a license, each company applicant may file a certificate of insurance in the amount of $100,000 in lieu of the individual insurance of its employees.
(C) Annual renewal of insurance. The bond or certificate of insurance shall be renewed and filed annually with the application for renewal of the license or permit.
