§ 91.54 REQUIRED INFORMATION FOR COMPLAINTS.
   All complaints will require the following information to be recorded in order for the complaint to be processed:
   (A)   Name of the person filing the complaint;
   (B)   Address and phone number of person filing complaint;
   (C)   Location of hazard or violation; and
   (D)   Type of problem, hazard or violation.
