§ 112.04 FILING AND CONTENTS OF APPLICATION.
   Application for a license under this chapter shall be made, in duplicate, to the City Clerk upon a form supplied for that purpose by the City Clerk. Such application shall contain the following information:
   (A)   The name, address, age, date, and place of birth of the applicant;
   (B)   Prior convictions of the applicant, if any;
   (C)   The place where the family amusement center is to be located; and
   (D)   A description of amusement and recreational devices to be located in the family amusement center.
