(A) The form by which a person applies for a position shall require:
(1) Information about the applicant's training, experience and character; and
(2) Whatever additional information is required for an evaluation of the applicant's fitness for the position for which he or she applies.
(B) Each application shall be reviewed by the appropriate department head who makes recommendations to the Mayor.
(C) All application forms filed with the city shall be kept for the time required by law.
