§ 154.021 FINAL PLAT.
   (A)   The final plat shall be drawn in waterproof ink on Mylar sheets 17 inches by 22 inches with one-half-inch margins and a scale of one inch equals 100 feet or larger. More than one sheet with careful matching should be used where necessary rather than the use of small scale maps.
   (B)   The final plat must show the following:
      (1)   Proposed name of the subdivision;
      (2)   Location by township, or other legal description;
      (3)   Names and address of the owner, developer, and surveyor or engineer who made the plat;
      (4)   Date, north point, and graphic scale;
      (5)   Boundary of plat with angles and dimensions based on an accurate traverse;
      (6)   Exact location, width, and name of all streets and alleys within and adjoining the plat;
      (7)   True angles and distances to the nearest established street lines or official monuments which shall be accurately described on the plat;
      (8)   Where appropriate, county, township, or municipal lines accurately tied to the lines of the subdivision by distances and angles;
      (9)   Complete curve data for all curves included in the plan;
      (10)   Location, dimensions, and purpose of all easements, and profiles and cross-sections of streets;
      (11)   Lot numbers and dimensions;
      (12)   Accurate dimensions of any areas to be dedicated or reserved for public, semi-public, or community use;
      (13)   Building setback lines show with dimensions;
      (14)   Location and description of all monuments;
      (15)   Certificate of surveyor as to accuracy of survey and plat, with the signature and seal of the professional engineer or registered land surveyor responsible for the preparation of the plat;
      (16)   Approximate boundaries of areas subject to periodic flooding or overflow of stormwaters and marshy areas;
      (17)   Statement of owner dedicating rights-of-way sites for public use;
      (18)   Restrictions of all types which will run with the land and become covenants in the deeds for lots; and
      (19)   Certificate for approval by the Planning Commission.
