4-3-3: DEVICE LICENSE REQUIREMENTS:
   A.   Application For Licenses; Fees:
      1.   Within five (5) days after any duly licensed operator shall display any amusement device or musical device or public pool or billiard table for use by the public, such operator shall file with the clerk an application for a license for each such device. The application for a device license shall be properly sworn to under seal and oath, with a brief description of each such device and the premises where such device is located, together with the such other relevant data as the council may require. Each application for a device license under this subsection A1 for any amusement device or musical device or renewal thereof shall be accompanied by an annual license fee as provided in section 3-2A-1 of this code. Each application for a device license hereunder for a public pool or billiard table or renewal thereof shall be accompanied by an annual license fee as provided in section 3-2A-1 of this code. The city shall issue for each such device on which an application is approved a license stating the year for which it is issued. The city reserves the right to deny or to revoke the license of any such device which cheats its patrons of money or amounts to a gambling device.
      2.   Within five (5) days after any duly licensed operator or any person, partnership, corporation or association shall display any vending device for use by the public, such operator or any person, partnership, corporation or association shall file with the clerk an application for a license for each such device. The application for a device license shall be properly sworn to under seal and oath, with a brief description of each such device and the premises where such device is located, together with such other relevant data as the council may require. Each application for a device license under this subsection A2 or a renewal thereof shall be accompanied by an annual license fee as provided in section 3-2A-1 of this code. The city shall issue for each such device on which an application is approved a license stating the year for which it is issued.    B.   Transfer Of License: All device licenses herein provided for shall be transferable from one device to another device, regardless of whether the devices are of the same type. Any such transfer must be reported within two (2) working days from the date of transfer, in writing, to the city. The notification of transfer must include the name, serial number and location of each device or table transferred. (Ord. 2000-O-49, 8-15-2000)
