143.11 OFFICIAL'S LICENSE OR PARTICIPANT'S OR PROFESSIONAL BOXER'S LICENSE REQUIRED; APPLICATION.
   Any person who desires to participate in boxing as a referee, judge, matchmaker, timekeeper or professional boxer, or as a manager, trainer or second of a professional boxer, except a second to fewer than three boxers, shall apply for a license from the Boxing Commission. The application shall be on forms provided by the Commission. Each application shall be accompanied by the license fee prescribed in Section 143.13. The applicant shall verify the application under oath.
   The Commission shall prescribe the form of the application for a participant's license. The application shall include the correct and ring or assumed name of the applicant, the applicant's address, the applicant's date and place of birth, the applicant's occupation, two sets of the applicant's fingerprints and a copy of the applicant's win and loss record as a professional boxer.
   An application for a professional boxer's license shall also include a certified copy of the results of a physical examination of the applicant that a licensed physician conducted not more than thirty days prior to the filing of the application.
