719.03 APPLICATION FOR EXHIBITOR'S LICENSE.

   Application for a license to display a mechanical or electromechanical amusement device shall be made to the Mayor upon forms furnished therefor by the Mayor. Such application shall be made by the owner or proprietor of the business or place at which such mechanical or electromechanical amusement device is to be displayed. The application shall state: the name of the owner of such place or business; the address of the place for which the license is sought; the residence address of the owner; the serial number and name of the manufacturer of the mechanical or electromechanical amusement device; the owner of such mechanical or electromechanical amusement device; the name and address of the distributor of such mechanical or electromechanical amusement device; the nature of the business in conjunction with which such mechanical or electromechanical device is to be displayed; and such other information as may be required by the Mayor.
