381.04 PERMIT APPLICATION AND FEE; ISSUANCE.
   (a)    Permits for establishing and maintaining a site shall be issued to the owner or lessee of the site by the Chief of Police within thirty days after the applicant has submitted a written application therefor, paid an application fee of twenty-five dollars ($25.00) and has submitted, in writing, the following information:
      (1)    Pilot qualification as evidenced by the proper FAA Certificate and a log book indicating logged number of hours of helicopter time. The Chief of Police shall consider a commercial helicopter pilot or a private helicopter pilot who has logged more than 200 hours of helicopter time as being properly qualified within the meaning of this section.
      (2)   Financial responsibility of the operator as evidenced by insurance policies or a certificate of insurance.
      (3)    Purpose and frequency of proposed operation as evidenced by a statement in writing by the operator.
      (4)    The site has been inspected, approved and registered or licensed by the Division of Aviation of the Ohio Department of Transportation for compliance with the criteria set forth in Ohio Administrative Code Chapters 5501:1-1, 5501:1-2 and 5501:1-5.
   (b)    Every permit issued under this section shall expire no later than ten days after the date of its issuance.
