709.14 REQUIREMENTS FOR PRIVATE SECURITY SERVICE LICENSE.
   (a)   Every applicant for a license as a private security service shall:
      (1)   Produce, on forms provided by the Chief of Police, the name in which the business is to operate and the address of the principal place of business;
      (2)   List the name and address of the owner (if a sole proprietorship), each partner (if a partnership) or each officer (if a corporation), together with the names and addresses of all shareholders owning more than ten percent of the issued and outstanding stock in the corporation;
      (3)   State the length of time such business has been in operation at all places where the business has been located during the last ten years;
      (4)   List all felony convictions over the past ten years of every owner, partner or corporation officer of the business; and
      (5)   Swear or affirm before a notary public that the information set forth in the application for such license is true.
   (b)   Failure to list a conviction or other material information contained on the application form shall be considered falsification of the application and shall be grounds for refusal to issue the license and/or shall be a violation of this section.
